[philmont] Medical form - vaccination dates

For the adults, on their medical form, does the medical staff @ Philmont
expect us to have the dates for our vaccinations we received as kids (40
years ago) ? I'd have to have my mother go and dig them out, assuming
she still has them.

I have my tetanus shot date but none of the others.
