I have been somewhat surprised by these responses. The literature sent
to a crew states pretty clearly (and in a few places, if I do not recall
incorrectly)
that only ORIGINALS will be accepted.

That said, our crew had one family that lost their form. Instead of asking
me
(as crew advisor) for another, they called Philmont and had a form faxed to
them. Philmont had no problem accepting their faxed form. I called them
when I found out about it and asked if it was okay.

However, I think what they want to see are original signatures, more than
original forms. I think you may run into some substantial issues at checkin
if you hand in photocopies of signatures.

Better option is to try to stick to originals, though.

AS ALWAYS, CHECK WITH PHILMONT BEFORE YOU LEAVE HOME.
They charge something like $300 to do a med check if you show up without
a form. That can make for a bad day.

The staff at Philmont are wonderful people and very patient with answering
your questions. The phone call is a lot less expensive than other
alternatives.
