Yes, please, please, please make sure the medical forms are filled out.
Every crew I took out had atleast one person who did not fill out the med
form completely. It defies all logic that it is not filled out. Most common
things left out:
1. religous preferance
2. emergency contact info
3. INSURANCE information (if there is no insurance don't leave it blank, put
none)
4. Copy of insurance card
5. Social Security number
6. INSURANCE information
7. Signature of youth
8. Signature of adult
9. INSURANCE information
Always there is someone in the crew who doesn't get something filled out.
Then little Johnny has to call home and ask mom or dad about it. Invariably,
mom and dad are using this chance as a vaction of their own and are not
reachable.
It will help speed you through base camp (especially med rechecks) if you
have your med forms filled out correctly and completely.
Oh and I almmost forgot, don't forget to make sure the INSURANCE information
is filled out ;)
