In 2002, we got all our med forms back when we picked up our patches and
pictures. I would be more concerned if the forms were lying around after we
left and didn't get properly disposed of (shredded)

Yeah, I know. My Dad is one of those who tells people about the Freedom of
Information Act whenever he gets the chance. Handing out SS# opens the
doors to identity theft. But if you think about it, when was the last time
you went to a Doctor's office or Hospital when they didn't ask for it? I
don't know the exact purpose for having it, but my guess would be if the
participant had to come off the trail and go to the hospital. It may be for
insurance verification. I don't know but I do know its on the form and needs
to be filled out. You can save time haggling once you get to basecamp if you
just do it and hope that those handling the med forms remember the 1st point
of the Scout Law.

It's your choice. Fill it out ahead of time or wait for Johnny to call home
and get it. Of course you may get lucky and the re-checker skips over it on
a busy day ;)
