One of the forms in our off-water packet at Northern Tier (Sommers Canoe Base) last summer was an application form for this award. I looked for it a bit this afternoon but couldn't lay my hands on it (probably in my office). I'm sure if you called the folks at Sommers Canoe Base ((218) 365-4811) they will help you out. Or you might check with the last High Adventure base you attend.
