Philmont requires most of the information on the Class 3
form and MORE. I've been told they will only accept the
2002 Philmont Medical form this summer.
I've been told that Philmont will not accept a "copy" or
a form which has been downloaded from a web page. They
want the ORIGINAL 2002 form which contains the "Risk
Advisory" information and the pages which describe "The
Philmont Trek Experience." Philmont also requires a copy
of the individual's insurance card. If the family has no
insurance, this must be stated on the form.
