The Philmont medical form is a special three panel wide document that has at
the top of the front page a title that says "Philmont Scout Ranch Boy Scouts
of America; 2002 Health and Medical Record". The remainder of the first
page and all of the second page gives warnings for such things as "Cardiac
or Cardiovascular Disease", and "Hypertension". The third and fourth pages
are the actual health record - which is straight forward and not complicated
at all. The fifth page is a place to list any medications used, and the
sixth page is a "Risk Advisory" and the "Philmont Weight Limits For
Backpacking & Hiking".
Having been the council contingent leader for nine years and been there for
the past eight, I can say the Philmont is very particular about the health
info. All questions are to be answered; and the physician's and parent's
signatures are a MUST. They will not let you out on trial without them.
I've have more than one leader spend most of his time in base camp phoning
home to speak with a parent to fax a permission back to Philmont. The copy
of the health insurance card is also required, and one person in a crew MUST
have a valid CPR card WITH them. They will not let you go out on trail
until one is produced.
All this is protection for the individual and Philmont. When you leave
Philmont, they return the physicals (some states such as NYS required that
they are retained - NM doesn't), and can be used for local camp medical info
forms (check local requirements).
Because of this - I have everyone - one by one show me his medical form with
signatures as they board the plane - or they don't get on.
