[Philmont]: High Adventure Application forms

Our troop is going to Philmont in 2006. We have enough experienced scouts
and leaders for a small crew (7), and will register as such, but we would
like to fill the crew if possible. Does anyone have a 'high adventure
application' form they use to help gather information on scouts and adults,
to help make the final decision on who the final crew members will be? This
is the third time our lead advisor has gone, although the first time he's
led a trek with this troop and is hoping this type of tool will be
helpful. We actually had one a few years back in our old troop, and it
worked well -- sadly, I can't find it. Any and all help/links etc. would be
appreciated! Thanks!
