There are some realities about "steep terrain" that are problematic. The best we can get, here in Dallas, is Winding Stair up in Oklahoma. There isn't that much "steep" to it really. Also, the real preparation should be in working at altitude. That isn't going to happen in the plains of the US either. So we have to make do with stadium stands and running and backpacking over the best we can get. If we lived near the Sierras or near the AT, things would be different. I'm sure this is the primary reason we spin it in the direction of exercise and focus more on skills and techniques.
