Re: [Philmont]: SSN on Medical Form

I would tell the parents the following.
1. The health form is covered by the HIPPA legislation. ( Medical Privacy Act)
2. The health form is used to care for their son.
3. The origional form is returned to the Lead Crew Advisor upon departure
from Philmont.
4. E-mail Philmont for their privacy statment and give it to the parents.
