Below is a simple form we used on our Trek and I have used for other Trips
as well. Not reviewed by a lawyer. I just wanted something that documented
an understanding we had with parents.
 
Permission for Non-Prescription Medication
 
 
In consideration of the communication difficulties and the possibility of
minor illness or injury inherent in the activity of backpacking,
 
I, _______________________________, authorize the administration of
        (Print Name of Parent or Guardian)
 
non-prescription medications for the treatment of minor illnesses or injury
 
to my son, ________________________________ while backpacking.
                (Print Name of Scout)
 
Such administration shall be at the discretion of the adult Scout Leaders
 
for the trip scheduled ________________________________________.
                                (indicate Dates of the Trip/ duration of the permission)
 
I have indicated any exceptions below.
