They also advise against butterfly closures, probably for same reason: can't get it clean enough in back country.

I would not. Condition are such that the Wilderness Medical Society also advises against it. There protocols do not come into play unless you are 6 hours from any medical care. Situation bad enough, flight for life from Taos can be there in less than an hour. Just wasted extra weight.
Also, I would suspect anything that needed suturing your not going to get clean enough in the back country and you will want to leave it open until you can clean it out.
