Contact the county in the state (usually health dept) where you born and find out the procedure on how they want to handle birth certificates. With identity theft issues they are a little reluctant and usually require a little proof and payment. (Driver's license, and a reason why) Some of them will allow it over the phone, some even have a website. Priority mail the proper documents and money to them and include payment for priority mail back (or express, FedEx etc) if you need them in a hurry.
If you want to get a passport, go online and figure out how to do that. Go to your local post office and get the form. Fill the form out completely and take the form, money, (and they have some funny money issues I can't recall right now, checks only I believe), birth certificate to the Post Office. Have the post office swear you in and sign the passport application.
If you need it ASAP, there are several companies you can find on line that will hand walk the application through. They usually charge about $250 on top of the passport costs. Normally mail and passport operations are about 8 weeks I think
