Any physical within one year of the trek is considered adequate. If
you're splitting hairs, I believe the one year is marked from the ending
date of your trek (that is, so that your physical doesn't "expire" while
you're still on the trail). In your case, a physical completed in
November 2003 is fine for a June 2004 trek.
However, there is a more important point - that being, Philmont will
accept ONLY the Philmont medical form, completed and signed by your
physician. You can't just staple a non-Philmont physical to the
Philmont form (yes, this includes the BSA Class III "High Adventure"
Physical form). There are NO exceptions. In case of doubt, refer to
the previous sentence.
Fortunately, most physicians are willing to complete the Philmont form
from a physical they've already completed within the specified time
frame. You may need to leave the form with him/her for a week so he/she
can do it when some time frees up. A self-addressed, stamped envelope
can save you a trip, so long as you trust the USPS....
