Steve,
I have found Philmont to be somewhat flexible and reasonable. If I
were in you situation I would make a photocopy of the forma and send
that to the families that I could not see at a face-to-face meeting
and have them sign (or decline) and send it back to you. I imagine
that Philmont will be able to accommodate a couple extra pieces of paper.

As for "how this affects you" - It is my understanding that they will
just avoid allowing publicity (like Boys Life) from photographing
your entire crew. They would still take the base camp group photo.
