[Philmont] : Crew member agreement

I haven't read it in a yr or so. But I plan to use it for my 2007 crew, and
may update a little.

In 2003 we sent a scout home from Base camp before the trek even started,
because he was not living up to his agreement in the C of C he and his
parents signed.
